By using Eligible Labs you agree to these terms. If you use it on behalf of a facility, you confirm you have authority to bind that facility.

Eligible Labs helps facilities manage long-term care Medicaid applications and renewals. It includes eligibility screening, document collection, case tracking and reminders.

Eligibility screens are a first pass based on published program limits. They aren't a determination of eligibility. State agencies decide eligibility. We don't guarantee that any application will be approved.

Keep login details safe. Enter accurate information. Only add people and records you're allowed to share. Get any permission needed before we contact a family member on your behalf.

You keep ownership of the information you put in. You give us permission to use it to provide the service, as described in the Privacy Policy.

Don't misuse the service, try to reach another facility's records, interfere with its operation or use it for anything unlawful.

Fees are set out in your agreement with us. Plans are described on the pricing page.

The service is provided as is. To the extent the law allows, Eligible Labs isn't liable for indirect or consequential damages, including lost revenue from a denied or delayed application.

You can delete your account at any time in settings. We may suspend access for a breach of these terms.

We may update these terms and will post the new date on this page.
